Foil Surfing Injury Survey

Thank you for taking the time to help record injuries from our sport. This data will be used to give a talk at The Surfers Medical Association Conference that is coming up in September.

There is a lot of misconception about foil surfing and how dangerous it is. I want to see if that is true or not relative to other sports.

All information will be kept confidential

No Names or identifiers except age and gender will be revealed during the talk.

Any Pictures used with be with expressed written permission and I will kept identification of the person confidential

I need as much information as you can recall. If you do not know it then do not worry about it. Please do not make things up. This is for research purposes.

·          Foil injury date,

       This can be just the month and year

·          Foil injury beach or surf break location

·          Foil injury body location

·                  Type of injury

             Laceration

             Bruise or contusions

             Fracture

             Sprain or Strain

             Concussion

·        Explanation of injury

         Leash under foot caused…

         Lost balance and landed on board edge or foil or etc

         Coming up from fall and hit wing

         Collided with surfer, fellow foiler

·        Treatment received

o   Did you self-treat?

o   Did you go to a medical facility?

·        How long did you recovery take?

·        How long until you foil surfed again?

Information about you

·        # of years foiling

·        # of hours per week foiling

·        Do you wear any safety gear? If so what?

·        Did the safety gear prevent an injury?

·        Do you know of any injuries to surfers or swimmers from a foil? Please explain
